Full course description
This Case Management course prepares BHA-I trainees at Wind River Family & Community Health Care to provide culturally grounded, community-based support within the context of the Wind River Indian Reservation. Throughout the course, trainees learn the foundations of case management; how to identify and connect clients to resources; how to make and follow up on referrals; how to document professionally and protect confidentiality; and how to support medication management, group participation, crisis response, discharge planning, and continuity of care.
